B4 School Check

The B4 School Check is a free wellness check for four-year-olds. It aims to identify and address any health, behavioural, social, or developmental concerns which could affect a child's ability to get the most benefit from school, such as a hearing problems or communication difficulty.

Care Plus

Care Plus targets people with high health needs due to chronic conditions, acute medical or mental health needs, or terminal illness. Care Plus aims to improve chronic care management, reduce inequalities, improve primary health care teamwork and reduce the cost of service for high-need primary health users.

Cardiovascular Disease/Diabetes

  • Cardiovascular Disease (CVD) and Diabetes Risk Assessment Project
    This project enables early identification and management of people at high risk of CVD and diabetes. Early identification of these people will prevent or reduce the impact of these diseases on people’s lives and reduce hospital admissions, premature mortality and morbidity.
  • Get Checked
    This programme enables our practices to offer their diabetes patients a FREE yearly check up, to help improve and monitor their condition.
  • Diabetes Self Management Education (DSME) / EPIC
    The DSME course gives people with diabetes education on what diabetes is, nutrition, foot care, and the changes and risks of type II diabetes. The service is delivered by a DSME coordinator over four-week courses throughout the year.
  • Diabetes Eye Screening
    This service provides eye screening for all diabetes patients to assess the retina of the eye and ensure there are no diabetes-related complications.
  • Podiatry
    Our Podiatry service aims to improve the condition of the lower limbs of all enrolled patients with diabetes and other chronic conditions, with particular emphasis on high needs patients. The service also aims to provide preventative measures, podiatric management and develop self-management strategies by encouraging independence with foot care.

Cervical Smears

We provide free cervical screening to women enrolled in a Waiora Healthcare practice. Cervical screening involves women having regular cervical smear tests to check for abnormal cell changes to the cervix (neck of the uterus or womb).

Green Prescription (GRx)

A Green Prescription (GRx) is a health professional’s written advice to a patient to be physically active, as part of the patient’s overall health management.

Health promotion

Our Health Promotion Plan gives us direction for achieving our community health objectives and highlights priority areas for working with our communities to promote targeted health programmes, such as Healthy Eating Healthy Action, B4 School Checks, One Heart Many Lives and Make A Stand.

Interpreting Service

We provide health interpreting and translating services for all languages. Please talk to your GP or nurse about this service.

Mental Health Programme

There are a range of services for people with mild to moderate mental illness who are able to receive care in a community setting. The treatment and social support options include medication advice/support, extended GP and nurse consults, phone based support, problem-solving, and access to whanau support services, cognitive behavioural therapy (CBT) community health coordinators and funding of transportation if required.

Nursing Home Visits

This programme provides nursing home visits for our enrolled patients, referred by their GP. Nursing Outreach Services includes in-home education, the prevention and management of long-term conditions, maternity, child and adolescent health, mental health, palliative care, services for older people and for people with disabilities.

Nutrition Service

The clinical nutrition/dietitian support service is designed to provide dietary advice and support to chronic disease clients or those at risk of developing/exacerbating chronic disease conditions.

One Heart Many Lives

One Heart Many Lives is a cardiovascular primary prevention programme, which targets Maori and Pacific men aged 35 years and older. One Heart Many Lives inspires men to take action and make changes which will improve their heart function and their long-term health. It encourages men to take long-term cholesterol lowering medication (and other heart medications) to decrease the effects of heart disease.

Outreach Immunisation Services

GPs advise Outreach Nurses of those children who are overdue for their immunisations, and the nurses follow up with families to make sure children receive their vaccinations in line with National Immunisation Register guidelines.

Palliative Care

Palliative care is the care of people who are dying from active, progressive diseases or other conditions that are not responsive to curative treatment. Palliative care embraces the physical, social, emotional and spiritual elements of wellbeing–tinana, whanau, hinengaro and wairua – and enhances a person’s quality of life while they are dying.

Primary Options for Acute Care (POAC)

Primary Options for Acute Care is a service that allows GPs to offer various aspects of care or treatment not usually available or affordable to the patient. Patients who might otherwise have to go to hospital can be safely treated in the community, under the responsibility of their GP.

Sexual & reproductive Health – under 22’s

This service provides free treatment and advice on sexual and reproductive health for under-22 year-old Maori, Pacific and low income patients. It includes contraception, sexually transmitted infection screening and health education.

Smoking Cessation

This service provides advice and support for people who wish to quit smoking.
